Whats the difference between the Old & New Republic?

The prequel Republic is known strictly as the Republic. By the time of A New Hope, it's known as the Old Republic since it's now been transformed into the Galactic Empire. After Return of the Jedi, Leia helps with setting up a 'New Republic', no doubt to reduce confusion.
