Well there's a couple of lines from the first trilogy but it's pretty scant offerings.

There is a 'fan fiction' novella on www.episode-x.com that has this entire scene in it, but it's just one person's take on it.

Yep a few lines in the ROTJ novelisation. Someone here quoted them a while back but I can't remember.

I always thought they fought and Anakin was pushed or fell into a pit of lava. I've thought this since I was a kid and I'm sure I heard it/read it somewhere...could it have been the original novel by George Lucas? I haven't read that since the seventies.

No, in general I think you're right. There was a paragraph or two about a lightsaber duel between Anakin and Obi-Wan, that supposedly ended with Anakin falling into a pit of molten metal or a reactor (not a volcano, I think it was industrial in nature) and Obi Wan trying to rescue him.

I don't know what book it was in though - and I always wanted to know more about that too. I suggest you check out that link I gave
